These draft terms cover use of Buela Physical Therapy's appointment and patient portal. Read the Privacy Policy for information handling.

Your account

Provide accurate details, keep your password and verification codes private, and use only your own account or an account you are authorized to create as a parent or guardian for a minor. Do not impersonate anyone, upload unlawful material, or try to access another person's records. Tell the clinic about suspected misuse. Verification confirms control of contact channels; it does not establish guardianship or replace clinical consent.

Bookings, cancellation and HMO

A submitted booking is pending until clinic staff approve it. Available services, clinicians and times may change; review your notifications. Use the appointment cancellation request with a reason and contact the branch if a visit is imminent. PT bookings require an approved referral. HMO selection does not guarantee eligibility, coverage, payment or a partnership; the clinic and provider must confirm these separately.

Care and emergencies

The portal is an administrative tool. Messages and displayed information are not emergency services, a diagnosis or a substitute for a consultation. For an emergency, contact local emergency services or go to the nearest emergency department. Do not delay care while waiting for a portal reply. Treatment results vary and are not guaranteed.

Availability and responsibility

We aim to keep the portal accurate and available, but interruptions and errors may occur. Contact the branch to confirm urgent arrangements. Any limits on liability apply only to the extent permitted by law; these terms do not waive mandatory patient or consumer rights, or excuse liability that cannot lawfully be excluded.

Changes and contact

The clinic will publish revised terms with a new version/date and explain material changes before applying them where required.
